The best way to make an appointment is to call at the number listed above. I generally take patients
Monday through Friday, but I do not post or keep regular office hours. This is done to create the greatest
flexibility in meeting the scheduling needs of my patients from week to week. I also do not take "walk-in" appointments,
but I am happy to accomodate "same-day" appointments whenever my schedule allows.
After scheduling an appointment, you will want to fill out a
New Patient Health History Form.
This takes approximately 30 minutes to fill out. Please bring the completed form with you to your first appointment.
